Ordering and payment alternatives are:

   Either use this form (page) or attach the Printable Order Form (above) to an email, and include credit card details (either in the box below, or via separate email).
2. Order offline
   Print the Printable Order Form (above) and either fax or post it, together with your preferred payment option. (Credit card details can be included on fax, or post us a cheque.)
3. Combination of on- and off-line
   Complete this form (page) and submit it, or email your order (attach our Printable Order Form above) and fax or post it to us, either with your credit card details or a cheque.

  Type of card - Bankcard/Mastercard/Visa; Name on card; Number of card & Expiry date. (Remember, this is not a secure site - use this method of payment at your own risk.)

How will you be paying? I am ordering online and will send payment or payment details by email, fax or post. I will send my order and payment (or payment details) by post. I am both ordering and paying online.
